This form is intended to provide the Ashborough Civic Association (ACA) Board of Directors with advice on residents of the Subdivision that are not in compliance with the Covenants and Restrictions approved and adopted by the Association. The form should be completed in its entirety submitted.

The submission of the form is on an advisory capacity to the Board of Directors and is not to be used as a notification to the alleged violator or to assume any action to be taken by the Board of Directors. As the handling of violations can be confidential, there should be no expectation to follow up to the completer of this form on the results or nature of action(s) taken by the Board.
